ADDITIONAL INFORMATION AND DOCUMENTS REQUESTED
1- Importer/manufacturer data sheet which is located in belove. (The competent authorities of the People&＃39;s Republic of China that will be held by the institution official content after approval, will be confirm diplomatic representatives in our country.
2- The manufacturer&＃39;s trademark registration documents if you have. (After the approval of the competent authority of the People&＃39;s Republic of China, both Chinese and Turkish documents and a certified translation into Turkish will be commissioned in the People&＃39;s Republic of China representative offices in our country will be confirmed.
The producer company who produce product a brand in our country Should have copy of trademark registration and be aware of other commercial documents/knowledge between these two company.

Bu form be? b?lümden olu?makta ve ihracat?? firmaya, üretici firmaya ve ilgili ürüne ili?kin bilgiler i?ermektedir. Bu formda verilen bilgilerin i?eri?inin do?rulu?u, ilgili ülkedeki resmi bir kurum taraf?ndan onaylanacakt?r. Formun tüm sayfalar?nda, üretici firman?n ve resmi kurumun yetkililerinin imzas? bulunacakt?r. Ayr?ca, resmi kurum yetkilisinin imzas? Türk Konsoloslu?u taraf?ndan onaylanacakt?r. ?retici firman?n beyan?, resmi kurumun i?erik onay? ve Türk Konsoloslu?u imza onay? B?lüm 5’te yer alacakt?r.
(This form consists of five sections and contains information with regard to the exporter company, the producer company and the product concerned. The accuracy of the content of information in this form shall be approved by an official body in the related country. All the pages of the form shall include the signatures of the authorized persons in the producer company and the official body. Also, the signature of the official body’s authorized person shall be approved by the Apostille of the Turkish Consulate. The producer company’s declaration, the official body’s approval of the related content, and the Turkish Consulate’s Apostille shall appear in Section V.)
